Quadrant Health Group and its affiliated treatment centers are committed to protecting the privacy of your Protected Health Information (PHI). Federal law requires us to maintain the privacy and security of your PHI, provide you with this Notice of Privacy Practices, and comply with the terms of this Notice.

Your Rights

You have the right to:
• Inspect or obtain a copy of your medical records.
• Request corrections to your medical records.
• Request confidential communications.
• Request restrictions on certain uses and disclosures of your PHI.
• Request an accounting of certain disclosures of your PHI.
• Receive a paper copy of this Notice.
• Designate someone to act on your behalf when legally authorized.
• File a complaint if you believe your privacy rights have been violated.

Your Choices

In certain situations, you may tell us your preferences regarding how we share your PHI, including sharing information with family members or others involved in your care, disaster relief organizations, and fundraising communications. Except where otherwise permitted or required by law, we will obtain your written authorization before using or disclosing your PHI for most marketing purposes, the sale of PHI, or most disclosures of psychotherapy notes. You may revoke an authorization at any time in writing.

How We May Use and Disclose Your Protected Health Information

We may use and disclose your PHI for:
• Treatment – coordinating and providing your healthcare.
• Payment – verifying insurance, billing, and obtaining payment.
• Healthcare Operations – quality improvement, compliance, accreditation, staff training, risk management, care coordination, and business administration.

• Medical examiners, coroners, and funeral directors

Our Responsibilities

Quadrant Health Group is required to:
• Maintain the privacy and security of your PHI.
• Provide you with this Notice.
• Follow the privacy practices described in this Notice.
• Notify you if a breach occurs that may have compromised the privacy or security of your PHI.
• Comply with applicable federal and state privacy laws, including HIPAA.

We will not use or disclose your PHI in ways not described in this Notice without your written authorization unless otherwise permitted or required by law.

We will not retaliate against you for filing a complaint or exercising your privacy rights.
